ABSTRACT

Small integral membrane protein 1(SMIM1) gene encodes Vel blood group antigen, and homozygous deletion of 17 bases pairs in its exon 3 leads to Vel negative phenotype. As a rare phenotype, accurate identification and prevention of transfusion reaction for Vel negative phenotype is particularly important. In addition to serving as a Vel blood group antigen, recent bioinformatics analysis suggests that SMIM1 protein may be a new tumor biomarker involved in the occurrence and development of numerous tumors. Due to the similarity in structure between SMIM1 protein and glycoprotein, and its phosphorylation in red blood cells infected with Plasmodium falciparum, it is speculated that SMIM1 protein may be involved in the development of malaria. Therefore, this article provides a review of the related research on SMIM1 and its coding protein in Vel blood type, tumor and plasmodium infection.

ABSTRACT

【Objective】 To learn more about the role of therapeutic plasma exchange in the management of cytokines release syndrome(CRS) after chimeric antigen receptor T(CAR-T) infusion by reviewing and analyzing the diagnosis and treatment of one case. 【Methods】 The diagnosis and treatment of lymphoma patients with CAR-T infusion related CRS were described, and case analysis was carried out by searching PubMed, Elsevier, Wiley, CNKI, and other databases for relevant guidelines, clinical trials, and case reports. 【Results】 The patient was diagnosed with follicular cell lymphoma. Progressive disease(PD) was assessed after multiple courses of treatment, and anti-CD19/20 CAR-T cell therapy was administered.The patient developed a high fever and chills, secondary dyspnea and hypotension at night on the day of infusion, and the inflammatory factors such as C-reactive protein(CRP) and interleukin-6(IL-6) increased sharply, suggesting the occurrence of cytokines release syndrome(CRS). After the patient was given symptomatic antipyretic, broad-spectrum anti-infection, tumor necrosis factor(TNF) antibody and three occasions of plasma exchange, the clinical manifestations of CRS gradually relieved. Three months after discharge, the patient was in complete response(CR). 【Conclusion】 CAR-T-associated CRS is a serious cellular immunotherapy-related toxicity that can result in multiple organ failure or even death in patients. Therapeutic plasma exchange may be a potential treatment for some patients with severe CRS.

OBJECTIVE@#To study the serological, molecular and genetic characteristics of an individual with para-Bombay blood group.@*METHODS@#Serological method was used to detect the presence of A, B, H antigens in red blood cells and saliva, and Sanger sequencing was used to analyze the FUT1 gene of the proband and her family members. Genetic mechanism of the blood group was analyzed by pedigree analysis.@*RESULTS@#Forward and reverse typing of the ABO blood group were inconsistent for the proband. A, B and H antigens were not found on erythrocytes, while B and H antigens were found in saliva, in addition with unexpected antibodies. The proband was found to have a genotype of ABO*B.01/ABO*O.01.04 caused by homozygous variant of c.948C>A (p.Tyr316Ter) of the FUT1 gene.@*CONCLUSION@#A novel para-Bombay blood group was identified, which was due to the missense variant of c.948C>A in the coding region of the FUT1 gene, which has probably resulted in inability to synthesis active H antigen transferase.

ABSTRACT

【Objective】 To study the serological and genetic characteristics of a case of B(A) blood group. 【Methods】 Serological and genetic ABO blood group typing were used to analyze the ABO subtype and family inheritance of the probands and her 8 family members. The B(A) blood type sample was used as the blood recipient, and the B-type and O-type donors were selected for cross-matching using microcolumn gel anti-human globulin method to evaluate the blood transfusion strategy. 【Results】 5 out of 9 family blood samples were B(A) phenotype, carrying B(A)04 allele. Among them, 1 was B(A)04/O1 type, and 4 were B(A)04/B type. The primary blood matching of B(A) blood type samples with type B and O recipients were all negative. 【Conclusion】 A total of 5 cases of B(A)04 blood type were found in this family investigation, and there were differences in serological manifestations. Washed RBCs with B and O type can be used for B(A) blood type transfusion, and type B suspended RBCs can be considered in case of emergency.

ABSTRACT

【Objective】 To investigate the impact of ABO blood group compatibility and incompatibility(major /minor/bidirectional incompatibility) on the outcomes of patients underwent allogeneic hematopoietic stem cell transplantation(allo-HSCT), and to provide evidences for optimizing the transplantation program. 【Methods】 From January 2014 to June 2018, we retrospectively reviewed the clinical courses of 18 recipients of allo-HSCT from ABO-compatible donors and 52 from ABO-incompatible donors at our hospital. The implantation time of granulocyte/erythrocyte/megakaryoblast, RBC/platelet transfusions within 3 months posttransplantation, the initiating and completion time of ABO blood group conversion(for ABO-compatible donors only) were analyzed and compared among the ABO-incompatible and ABO-compatible donors as such variables including demographic data, donor and patient relationship, diagnosis of disease, bone marrow hematopoietic function prior to transplantation, HLA matching were not significant different. 【Results】 For 18 recipients of allo-HSCT from ABO-compatible donors, the implantation time of granulocyte, erythrocyte, megakaryoblast was 12.0(11.0~16.3), 41.5 (35.0~49.0) and 19.0(16.0~22.5)days, respectively. For 52 recipients of allo-HSCT from ABO-incompatible donors, the ABO blood group conversion was initiated at 28.0(22.5~44.0)days posttransplantation and completed at 105.5(85.0~141.8)days. In the ABO-compatible group, the time of erythrocyte implantation was shortened(P0.05), no significant difference was observed between these two variables. The blood group conversion time, implantation time of granulocyte/erythrocyte/megakaryoblast, and RBC /platelet transfusions among ABO major, minor and bidirectional incompatible groups were not significant different (P>0.05). 【Conclusion】 ABO-compatiblity enjoys priority in allo-HSCT. ABO-incompatiblity can be chosen in the order of minor, major and bidirectional incompatibility in the absence of ABO-compatiblity.

ABSTRACT

With continuous discovery of tumor immune targets and continuous changes in antibody research and development technology, antibody drugs are becoming more and more widely used in clinical practice. However, some targets are not only expressed on tumor cells, but also on red blood cells. Therefore, the clinical application of antibodies against the corresponding targets may interfere with the detection of blood transfusion compatibility, resulting in difficulty in blood matching or delay of blood transfusion. This consensus summarizes the current solutions for the interference of CD38 monoclonal antibody (CD38 mAb) in transfusion compatibility testing. After analyzing the advantages and disadvantages of different methods, polybrene and sulfhydryl reducing agents [dithiothreitol (DTT) or 2-mercaptoethanol (2-Me)], as a solution for CD38 mAb interference in blood compatibility testing, are recommended for Chinese patients, so as to eliminate blood transfusion interference produce by CD38 mAb and further provide a pre-transfusion workflow for clinicians and technicians in Department of Blood Transfusion.

ABSTRACT

【Objective】 To evaluate common laboratory items in a large-dose blood loss model in vitro using thromboelastogram (TEG), to provide a reasonable infusion solution for clinical massive transfusions. 【Methods】 On March 2nd, 2017, eight healthy blood donors who participated in voluntary blood donation in the Department of Blood Transfusion Medicine of the First Medical Center of the PLA General Hospital were selected to undergo phlebotomy, and an in vitro dilution model of massive blood loss was established based on the previous research, namely Model 1 (M1, given massive transfusion protocol) and Model 2 (M2, given packed red blood cells and plasma) were established. Then blood routine, routine coagulation function, clotting factor activity, TEG of each model were tested. 【Results】 The platelet count in the M1 model was 61.00±10.24 (×109/L), and reduced to 28.83±10.36(×109/L) in M2 (P<0.01). The MA value (mm) of two groups detected by TEG was 29.35±2.37 vs 20.53± 2.76 (P<0.01). In M1 and M2 model, The activities of primary clotting factors respectively decreased to about 40% and 30% to original in M1 and M2. The R value of TEG prolonged to (6.32±0.85) min and (7.27±0.63) min respectively, still within the normal range(baseline 4.97±1.04). The fibrinogen concentration in the M1 and M2 model decreased to (1.10±0.08) g/L and (0.81±0.10) g/L(P<0.01), which had the same variation tendency to Alpha angle of TEG (25.65±4.95 vs 16.63±3.94, P>0.05). 【Conclusion】 MTP with blood components supplemented such as platelet and cryoprecipitate in time has effectively improved the Plt and Fib in vitro large-dose blood loss/transfusion model.

ABSTRACT

【Objective】 To investigate the preoperative anemia and perioperative blood transfusion in patients with duodenal papillary carcinoma who underwent Whipple surgery. 【Methods】 The clinical data of 1 959 cases with duodenal papillary carcinoma, subjected to Whipple surgery, were retrospectively analyzed. 【Results】 The rate of anemia in preoperative patients with duodenal papillary carcinoma was 54.87%(1 075/1 959). The incidence rate of anemia in the three age groups from low to high was 44.92% (≤50 years old, 190/423), 52.82% (51~64 years old, 506/958), and 65.57% (≥65 years old, 379/578) (P<0.05), and the highest rate of anemia occurred in patients aged above 65. There was a significant statistical difference among patients with different body mass index (BMI)(P<0.05). Patients with moderate or severe anemia received more red blood cells than patients with mild anemia during the perioperative period (P<0.05). The average hospitalization time of the blood transfusion patients was 27.25 days, and that of non-transfusion patients was 22.22 days (P<0.05). The amount of blood loss and hospitalization time of patients underwent laparoscopic and robotic surgery were significantly lower than those underwent open surgery patients (P <0.05). There were only 24.09%(186/772) treated with drugs for anemia intervention and the majority of patients (75.91%, 586/772) were treated with blood transfusions to interfere with anemia during hospitalization. 【Conclusion】 There are significant differences in the incidence rate of preoperative anemia among patients with duodenal papillary carcinoma who undergone Whipple surgery. Low BMI, abnormal WBC, and perioperative blood transfusion are high-risk factors for prolonged hospital stay, whereas anemia is not associated with prolonged hospital stay.

Objective To investigate the value of anti-HI antibody screening and identification in clinical blood transfusion . Methods 4 cases of anti-HI antibody positive in our hospital from February 2016 to November were selected as the research subjects .The irregular antibodies screening and identification were performed by adopting the anti-human globulin and saline test tube method ,then the blood transfusion effect was evaluated .Results Four cases were ABO blood group ,in which 2 cases were group A RhD positive and 2 cases were group AB RhD positive .The screening results of anti-human globulin method were weak positive , while which of saline test tube method were positive .Under the saline medium condition ,the reaction results of serum with 16 spectrum cellular reaction in 4 cases were positive ,while which with autoerythrocytes were negative ;under the anti-human globulin medium condition ,the reaction results of serum with 16 spectrum cellular reaction in 4 cases were negative except 1 case of weak positive ,while which with autoerythrocytes were negative .After blood transfusion in 2 cases of treatment blood use ,Hb increase reached the expected effect without blood transfusion adverse reactions occurrence .Conclusion In blood transfusion for the patients with anti-HI antibody positive ,the blood of same type ABO and compatible results in both saline test tube method and anti-human globulin tests should be selected .

ABSTRACT

Objective To analysis the related influencing factors and explore the approach of improving the effect of platelet transfusion through compared the posttransfusion effect of matched-type with random platelet transfusion.Methods Analysis the results of platelet antibody screening test and matched-type test in our hospital′s inpatients during July 2013 to June 2014,who applied for platelet transfusion.The influence of sex,blood transfusion history,pregnant history on the antibody were analyzed.The factors of sex,times of blood transfusion,pregnant times,platelet component sorts,storage time,combined with other components,platelet antibody (positive or negative) and transfusion matched-type platelet on the effect of platelet transfusion were analyzed.Application in our clinical blood transfusion intelligent management and evaluation system to evaluate the effect of platelet transfusion.Results Totally 812 patients′ platelet antibody were screened.Then we randomly selected 87 antibody positive inpatients were selected and 1 247 U plateletswere transfused,which including matched-type platelet transfusion for antibody screening positive patients,random platelet transfusion for antibody screening positive patients,random platelet transfusion for antibody screening negative patients,there were statistical difference (P<0.05).With Logistics regresion analysis,the history of blood transfusion was an independent risk factor for platelet antibody production (P<0.05,OR=13.104,95%CI:7.784-22.061).Sex (P<0.05,OR=1.629,95%CI:1.236-2.148),transfusion times,different platelet component sorts (leukocyte-reduced platelets aphaeresis and irradiation leukocyte-reduced platelets aphaeresis),different storage time,transfusion combined with other components (RBC)(P<0.05,OR=2.464,95%CI:1.053-5.765),transfusion matched-type platelet(P<0.05,OR=0.576,95%CI:0.389-0.854) were the risk factors for platelet transfusion.Conclusion Matched-type test should be done to improve efficiency when the platelet antibody screening were positive.Sex,times of blood transfusion,platelet component sorts,storage time,combine with other components,transfusion matched-type platelet couldinfluence the effect of platelet transfusion.

ABSTRACT

Objective To contrastively analyze the performance of two kinds of blood cell separators for apheresis platelets . Methods 80 blood donors were selected from 4 234 healthy apheresis platelet donors and their data before and after apheresis platelets were respectively collected by using two kinds of different blood cell separators Trima and Amicus .The product quality during the collection process ,residual WBC and RBC count ,acquisition time ,collection efficiency ,scrapping situation and safety were performed the comparative analysis between the two kinds of blood cell separator .Results In the case of the difference of the basic parameters before apheresis platelet having no statistical significance ,the two kinds of blood cell separator had no differences in the aspects of product quality ,processing blood amount and the use amount of anticoagulants(P>0 .05);but there were statisti‐cal differences in the aspects of acquisition time ,collection efficiency and pipeline residual blood amount (P<0 .05) .Conclusion Two kinds of blood cell separators have their own advantages and disadvantages during the use process ,so the blood cell separator should be selected according to the physical quality and their own characteristics of blood donors .

ABSTRACT

Objective To evaluate the transfusion efficacy and safety of patients with non‐specificity irregular antibodies .Meth‐ods A total of 19 cases of patients with non‐specificity irregular antibodies were analyzed ,then the transfusion efficacy and the in‐cidences of adverse transfusion reactions were investigated .Blood donor samples with the same ABO blood type of patients were randomly collected ,then blood cross‐matching was carried out .And the transfusion efficacy and safety of patients with non‐specific‐ity irregular antibody were evaluated when compatible blood were found by using both micro‐column gel technique and saline meth‐od .Results The non‐specificity irregular antibodies of the 19 cases of patients all were IgG irregular antibodies .The erythrocyte transfusion was carried out in the 19 cases of patients ,the total efficiency rate ,partial efficiency rate and inefficiency rate was 73 .7% ,26 .3% and 0 .0% ,respectively .No delayed hemolytic transfusion reactions were observed in any patient .Conclusion For patients with non‐specificity irregular antibodies ,the blood transfusion could be efficacy and safety when compatible blood samples are confirmed by using both micro‐column gel technique and saline method .

Objective Serological characteristic analysis for patients with autoimmune hemolytic anemia(AIHA)who has cold and warm auto‐antibody when undergoing blood type and cross‐matching .Methods Choosed 3 donors of red blood cells of group O to absorb autoantibodies of the patient′s serum(by using the methods of cold absorption) and used 2‐mercaptoethanol(2‐Me) to de‐stroyed the IgM antibody .Results The patient of AIHA blood type was B+ .There was cold and warm auto‐antibody in the pa‐tient′s serum ,and the result of cross‐matching is incompatible .Conclusion The patient′s serum contains cold and warm auto‐anti‐body which interferes blood type and cross‐matching .In emergency ,considering the patients condition ,blood transfusion combined with the drug treatment could achieve good effects .

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the value of three preoperative imaging methods in the anterolateral thigh flap (ALT) transplantation.</p><p><b>METHODS</b>According to preoperative imaging, patients who underwent the ALT flap transplantation were divided into three groups: computed tomography angiography (CTA) group, digital subtract angiography ( DSA) group and magnetic resonance angiography (MRA) group. There were fifteen cases in each group. We compared the imaging quality of the ALT artery among these groups and recorded the parameters of lateral femoral circumflex artery, descending branches and perforators including type, course and size. The results from images were compared with intraoperative findings. The success rate and complications were also recorded.</p><p><b>RESULTS</b>The preoperative imaging accuracy of the types of the lateral femoral circumflex artery and descending branch was more than 92.3%, with no significant different between any two of three groups (P > 0.05). The difference in diameters of descending branches and perforators from preoperative measurement and from intraoperative measurement was also not significant between any two of the three groups; the success rate and complications were not also obviously different (P > 0.05 , P > 0.05, respectively).</p><p><b>CONCLUSIONS</b>Preoperative mapping using CTA, DSA and MRA is a feasible and reliable method for the flap design in ALT transplantation. As a preoperative evaluation means, CTA and MRA may replace DSA in the ALT transplantation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of total flavonoids of Astragali Radix (TFA) and calycosin on apoptosis induction and cell cycle in human erythroleukemia cell line K562.</p><p><b>METHOD</b>MTT assay was used to measure the inhibition effect on the proliferation of K562 cells cultured with TFA and calycosin. The effect of TFA and calycosin on cell cycle in K562 was detected by PI staining. The apoptosis induction effect was measured by Annexin V/PI double staining. RT-PCR was used to determine the level of Cyclin D1 mRNA in K562 cells after treated with TFA and calycosin.</p><p><b>RESULT</b>TFA and calycosin could inhibit the proliferation of K562 cells, the 50% inhibiting concentration of TFA and calycosin were 98.63 mg x L(-1) and 130.32 mg x L(-1) respectively. TFA and calycosin could not induce apoptosis in K562 cells, but could increase the number of cells in the G0/G1 phase. The level of Cyclin D1 mRNA in K562 cells decreased after treated with TFA and calycosin.</p><p><b>CONCLUSION</b>TFA could inhibit the proliferation of K562 cells, and attribute to arrest them in the G0/G1 phase and decrease Cyclin D1 mRNA.</p>

ABSTRACT

Progresses in the studies on chemical constituents and bioactivities of total flavonoids of Astragalus (TFA) were systemically reviewed in this article. The results of studies on their bioactivities show that TFA has antioxidant and antitumor activities, and strengthen the immune system. Further biological studies on the species in total flavonoids of Astragalus are needed for better medicinal utilization.

ABSTRACT

BACKGROUND: Disturbance of lipid metabolism is responsible for many kinds of diseases, and to screen safe and effective prescription for lipid reduction has clinical significance.OBJECTIVE: To study the influence of puerarin-arginine composition on lipid metabolism in rats.DESIGN: Comparative observing study based on animal model.SETTING: Experimental animal center and department of biochemistry in a military hospital of Chinese PLA.MATERIALS: This study was carried out at the Experimental Animal Center and Department of Biochemistry of the General Military Hospital of Chinese PLA between May and August 2004. Eighty Wistar rats were randomly divided into high dosage, middle dosage, low dosage group and control group with 10 male and 10 female rats each.METHODS: Rats took food freely, and experimental groups were supplemented with puerarine-arginine composition of different dosage (0, 0.93,1.39 and 1.85 g/kg) for observing the influence on rats.MAIN OUTCOME MEASURES:① blood parameters; ② change of body mass and utilization rate of food; ③ parameters of vital organs;④histopathological changes.RESULTS: From onset of the second week, body mass of both male and female rats in experimental groups were found obviously lower than those in control group. The levels of blood cholesterol in female rats were(1.63 ±0.20) and (1.29 ±0.23) mmol/L in 0.93 g/kg dosage group and control group respectively, and were(1.75 ±0. 13) and(1.31 ±0. 19) mmol/L in male rats,and both have statistical significance( F = 6.43, 6. 58, P < 0. 05).Fatty parameters of the three experimental groups were found obviously lower as comparing with those of control group.CONCLUSION: Such composition has been proved to be able to exert function in lipid reduction by decreasing body mass, and improving utilization rate of food and proportion of cholesterol and organic fat.

BACKGROUND: Continuous oxygen inhalation of high concentration may improve antioxidase and simultaneously injure the lung in rats.OBJECTIVE: To observe the effect of intermittent high concentration oxygen on antioxidase activity and pulmonary tissue in rats.DESIGN: Complete randomized control experiment.SETTING: Department of Geriatric Cardiology and Department of Biochemistry, General Hospital of Chinese PLA. MATERIALS: The experiment was performed in Department of Hyperbaric Oxygen of Navy General Hospital and Department of Biochemistry of General Hospital of Chinese PLA from January 2002 to May 2002, in which, 10 healthy Wister rats were employed, mass weighted varied from 200 to 250 g, of either sex and randomized into two groups, 5 rats in each.METHODS: In the control, the rats were bred in the room environment for 7 days. In hyperoxic preconditioning (HP) group, the rats were placed in hyperbaric oxygen chamber and inhaled oxygen of 80% to 85% (101. 325 kPa) everyday for 6 hours, continuously for 7 days. At the end of experiment, the lung was collected to measure lung wet mass. 0.4 g pulmonary tissue was collected to prepare homogenate in freezing bath and its upper clear solution was collected to determine the activities of superoxide dismutase (SOD), glutathione peroxidase (GTTP) and catalase with chemiluminescence method successively. Pulmonary tissue was fixed with 30 g/L glutaradldehyde and prepared into sections to be observed the microstructure of lung under electron microscope.MAIN OUTCOME MEASURES: ① Comparison of lung wet mass and activities of SOD, glutathione peroxidase and catalase in pulmonary tissue in two groups. ② Alternation on microstructure of pulmonary tissue.RESULTS: Ten rats were all entered result analysis. ① Activities of SOD,GTTP and catalase in pulmonary tissue: Those in HP group were all higher remarkably than the control [(185.10±1.92) %/g, (90.50±4.17) nkat/g,(4.38±0.18) k/g; (95.15±1.43) %/g, (20.17±2.17) nkat/g, (1.10±0.19) K/g,t= -84.016, -33.488, -28.023, P < 0.01]. ② Lung wet mass: That in HP group was near to the control [(1.32±0.07), (1.30±0.03) g, P > 0.05], explaining that the fluid in the lung of HP group was not increased. ③ Microstructure of pulmonary tissue: In HP group, the basement membrane of pulmonary tissue was integrative and thickness was even, vascular endothelial cell was integrative and Ⅰ-type epithelial cell was not destroyed.CONCLUSION: Intermittent high concentration oxygen improves antioxidase activity in the lung of rat and does not injure the lung obviously.

ABSTRACT

Objective:To study an identifying model of HPLC fingerprint for the overall quality control of traditional Chinese medicine. Methods: A HPLC method is used to establish the standard fingerprint as well as sample fingerprint first. Then through the principles of numerical taxonomy and statistics, the similarity of the sample to the standard is tested by coefficients of cosine and distant, which are calculated with computer. Results: The combination of the two coefficients of cosine and distant can test the similarity of the sample to the standard correctly and accurately. Conclusion: The coefficients along with HPLC fingerprint can be applied to the on-line control in workshop to ensure the products' quality of all batches.

ABSTRACT

Objective: To establish the qualitative and quantitative standards for Yejuhua Granules (Flos Chrysanthemi Indici). Methods: The identification was carried out by TLC. Chlorogenic acid in preparation was determined by HPLC. The HPLC conditions were:Hypersil column (4.6nm?250 nm 5?L), mobile phase of acetonitrile-0.04% phosphoric acid solution (13∶87). Results: TLC method for the identification of Yejuhua Granules was specific. HPLC for the determination of chlorogenic acid in preparation was simple and quick, accurate and reliable. Conclusion: These methods can be used for the quality control of Yejuhua Granules.
